To schedule an appointment:

Please call our office at (229) 432-9325 to schedule your appointment or contact us completing our online form. We look forward to getting to know you, answering your questions, and helping you reach your plastic surgical goals.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment time to another patient.

Emergencies

If you have a major medical emergency, dial 911 and go to your nearest Emergency Center. After hours for urgent medical questions that cannot wait until normal office hours, you may contact us through the answering service by calling our office number.

Patient Forms

Download, print and fill out our patient forms before your appointment with Dr. Erhardt. By filling out these forms in advance, you will save significant time during your initial consultation visit. If you have any questions when filling out patient forms, please do the best you can – our staff will assist you with your questions on the day of your visit.  You may fax your forms to us at (229) 439-4396.
